The Incident Report, initially known as the Notebook, is a key item in Mario & Luigi: Brothership. It is a blank notebook found underneath a broken streetlamp to the left of Leyden's house on Bulbfish Island after the Mario Bros. hit a switch to turn on the streetlamps.
A security guard on the eastern part of the island asks three questions, one of which is "What is under the 'useless light'?" To answer it, the Bros. must investigate the area beneath the light, where they will find the Notebook, then show it to the security guard during the interrogation. After the Bros. successfully pass the interrogation, Snoutlet asks the guard what the Notebook was doing underneath the light. The security guard states that he always considered himself a detective and bought the Notebook to jot down his findings, but after he gave up, he buried it beneath the light. He then gives it to the Bros. as a present. Snoutlet then states the Notebook can become the Incident Report, adding that the investigation is up to them and that they should search the underground area.
By heading underground in the area the guard was blocking off, the Bros. and Snoutlet eventually come across an office. They find a diary belonging to Leyden's assistant, in which he rants about his grievances with his boss, further implicating him.
Once the Bros. return above ground and rest in the house Leyden lent them, Snoutlet summarizes the following facts that he wrote down in the Incident Report: Someone stole the island's prized Bulbfish eggs, so Leyden asked the Bros. to look into the case. Chilliam and Burnadette, an outsider couple visiting the island, were thought to be thieves by the islanders. In addition, several islanders were acting strange due to being possessed by Glohm. The security guard hired by Leyden's assistant was posted near the underground area's entrance, and the diary found there contained suspicious information.
When the Bros. exit the underground after Burnadette and Chilliam are arrested, they speak with the security guard to borrow a key. The guard approves, but on the condition that they get information from his younger brother, as he is a witness. To know of his whereabouts, the Bros. must meet with the secret snatcher by turning off the streetlights and heading to the island's northwest tip. There, they hand in the Incident Report to the secret snatcher, who shares information on the younger brother's location at the Anglerfish Restaurant, Chilliam and Burnadette's innocence, and the Extension Corps' Glohm ray.
Later, when the guard's younger brother reveals the culprit was wearing glasses and was therefore Leyden's assistant, Snoutlet mentions he made a note of it in the Incident Report.
